Root Cause Analysis & Apollo RCA
Luke has been using and facilitating Apollo Root Cause Analysis for over a decade. He has led numerous investigations, developed cause-and-effect charts, coached engineers and operational teams, and reviewed investigations across a range of industries and asset types.
His approach combines RCA with his reliability engineering background, helping ensure investigations are technically sound, evidence-based and focused on practical, sustainable solutions.
Luke also trains engineers, maintenance professionals, operational leaders and other technical personnel in the Apollo RCA methodology and the use of RC Pro software.

Luke is the owner and lead trainer at Root Cause Australia, delivering Apollo Root Cause Analysis training and professional RCA facilitation across Australia.
